Greyhound Jazz needed 300 stitches after being attacked near a children's playground while on a walk.
The six-year-old family pet also suffered a punctured muscle, broken tooth and damaged tongue, and will need more operations.
Now owners Stephen and Carol Ryles have called in Staffordshire Police as they demand for the dog's owner to be prosecuted.
Grandmother Carol, pictured above, aged 61, said: "I love animals but that should not have happened and what's to say that it won't happen again?
"That dog should have been on a lead and it should be put down. It was close to a children's play area and some of the children went to the vets to tell them what they had seen and they were in tears.
"I can't get the image out of my mind of what happened. We were always told if a dog wasn't on a lead it was because it was ok around other dogs."
Jazz was attacked as she was being taken for a walk on her lead near Biddulph Valley Leisure Centre, off Thames Drive, Biddulph, at about 6.40pm on Wednesday.
She was taken to the vets following the attack and transferred to another practice to be kept in overnight on a drip. The vets' bills have so far run into hundreds of pounds, although the couple are insured.
Mother-of-two Carol, of Oxhey Drive, Biddulph, added: "The English Bull Terrier came running down as dogs do when they see other dogs. At first it sniffed her for about a second and then it just attacked Jazz and was biting into her neck.
"She has three layers of stitches which run from her neck down her leg.
"She will need an operation in the future on her broken tooth and tongue. She might also have to have another operation depending on how the wounds heal. We are so grateful to the vets.
"At the moment she is crying a lot and is in a lot of pain. It has left us all really shaken and we are not getting much sleep. Jazz is a lovely dog and wouldn't harm a fly."
Retired factory worker Stephen, aged 61, added: "We have had dogs for years and I have never known anything like this."
Staffordshire Police have confirmed they are investigating the incident.
It is understood the owner has already checked with the vets' practice on Jazz's health.
