The report also says she had a compound skull fracture on the back of her head, an open wound on her face and bites on her back and under her right arm.
Her mother, 18-year-old Chelsea Lynn Brigman and her grandfather were both charged with involuntary manslaughter.
The narrative of the report says Brigman was inside her home with her father-in-law, two male friends and five dogs. Brigman told investigators that she went to take a shower, leaving Addyson in her car seat on a table with her father-in-law and the male friends.
Brigman said she was in the shower 5-10 minutes when the father-in-law called out her name and said, "Your baby!"
When she came back into the room, Addyson was on the floor face down in a pool of blood.
They then called 911 and started CPR. When EMS arrived and put her on the heart monitor, the report says she was "straight lined." The baby was pronounced dead at the hospital.
The report says Brigman told investigators the dogs were in a pen when she came to check on the baby. The wounds appear to be by animals, possible the dogs, the medical examiner says. But he adds he's not sure which dogs made the bites.
